An authorized person is whoever can sign for the LLC. For a single-member LLC that is usually just you, and one is all the state needs. Each person's name, address and title are printed on your Sunbiz filing and are public — that is why the address is asked for here as well as above.

You don't need an EIN to form your LLC — but you will shortly after, for banking. The IRS issues one free, directly, in a few minutes at irs.gov, once your LLC is approved.
